Important Note

The patient/insurance is not billed for these collections. Most of these orders come from Providence Sacred Heart Kidney Transplant Program, and the client billing account with that name should be selected when registering the patient in Epic. 

Order Kit Collection in Epic. We must have a diagnosis code and ordering provider name. 

Collection Instructions

Fill enclosed red top tube. Do not spin. 

Specimen Processing

Send the tube/kit to main lab specimen processing to be mailed out. 

Kit comes with the tube and cardboard box with the following address:
